Prepare For Your Visit
In order to facilitate your upcoming visit, please bear the following few steps in mind:
- Please bring proper insurance documentation and photo ID.
- Download all pertinent forms and fill them out as best you can prior to your visit. Turn these completed forms in to our office staff upon arrival.
- Try to avoid urinating just prior to your office visit. If you are waiting for an appointment and need to use the restroom, please notify the front desk staff.
- If undergoing an office procedure, please read the pre-operative and post-operative instructions for your procedure prior to your arrival.
- If this is your first visit, please bring a list of prescriptions with dosages that you are currently taking.
- Your co-payment is due at the time of your visit. We accept cash, checks, Visa, MasterCard and American Express.
- Late cancellation policy – We reserve the right to charge a nominal fee for appointments not cancelled within 24 hours.
- Please review all of our office policies prior to your visit.
IMPORTANT NOTICE
Effective 8-1-2009, the Federal Government is requiring physician practices to request and copy photo IDs from patients. This ID can be a valid US driver’s license or another government issued ID. We will copy your ID once per year but will request to see it at each visit.These new “Red Flag Rules” from the Federal Trade Commission are designed to help protect patients from identity theft. You may also be asked for personal identifying information over the telephone before discussing private patient information.
