Prepare For Your Visit

In order to facilitate your upcoming visit, please bear the following few steps in mind:

  1. Please bring proper insurance documentation and photo ID.
  2. Download all pertinent forms and fill them out as best you can prior to your visit. Turn these completed forms in to our office staff upon arrival.
  3. Try to avoid urinating just prior to your office visit. If you are waiting for an appointment and need to use the restroom, please notify the front desk staff.
  4. If undergoing an office procedure, please read the pre-operative and post-operative instructions for your procedure prior to your arrival.
  5. If this is your first visit, please bring a list of prescriptions with dosages that you are currently taking.
  6. Your co-payment is due at the time of your visit. We accept cash, checks, Visa, MasterCard and American Express..
  7. Please review all of our office policies prior to your visit.

In order to help protect our patients from identity theft and to preserve the integrity of their medical records, a valid driver’s license will be used as a part of our reliable verification process. Please bring your driver’s licenses or other form of government issued photo identification (i.e., passport) to all of your appointments.
