We will take cash, personal checks, and most major credit cards.
* Payment Plan Options - Please contact our Business Office for payment plans.
* Insurance Cards - Please bring a copy of your insurance card to every visit along with your photo ID.
* Demographic Information - Please verify your address, phone number and insurance at each visit.
* Participating Insurances - We participate with most major insurance plans in this area, including Anthem & Humana along with most Medicare Advantage products. Please contact us with any questions about participating plans. It is the patient's responsibility to know whether their physician is in their insurance's network.
* Cancellation of Appointments - Please notify our office in advance if you are not able to keep your appointment. There may be a fee if you do not notify us in advance.
