| Prior to all procedures the patient will be advised of eligibility, and their responsibility for the deductible. This is an estimate based on the scheduled procedure. |

| You will need to call your carrier and check to see if we are on the provider list. |

| You will receive an EOB (Explanation of Benefits) showing what the insurance is responsible for, insurance adjustments, non covered services and patient deductibles not met yet. |

| Along with co-pays may have a deductible which you are responsible for prior to insurance paying towards your balance. You also may have some non covered services. |

| Besides accepting all major credit cards, we offer Care Credit. Care Credit is a health credit card which we will be glad to discuss with you. |

| Please bring the following: a. Your insurance card b. A government issued picture ID (such as a driver’s license or passport) c. All you medications you are currently taking d. A list of your past surgeries, allergies and health history |

| Commonwealth Urology uses electronic medical records and all patient records are available to your physician regardless of what office you are seen at. |

| A urologist is a physician who specializes in the diagnosis, medical treatment, and surgical procedures relating to the kidneys, bladder, adrenal glands, and genitourinary system. Among the specialized areas with which urologists deal are infertility, prostate, incontinence, impotence, kidney stones, cancer treatment, vasectomies, vasectomy reversals, trauma to the urinary system, and pediatric care. |

| Our physicians are affiliated with most major hospitals in their regions. For a detailed list please see individual locations. |

| We will schedule all patients who call and request to be seen, however if your insurance requires a referral you will need to have that in file prior to your visit. Please check with your insurance for clarification. |

| Social Security numbers are used with insurance companies and laboratories as the identifier for all patients. The “Red Flag Rule” requires we verify all patients identity with a picture id. This is to protect the patient from someone else using their insurance plan and protecting their identity. |

| Medical records can be obtained by contacting our office and completing a "Request for Medical Records" form. |

| Medical records traditionally will be released within 7 days upon receipt of the release form. |

| All patients receive one copy of their medicals free of charge. Additional request will have a charge of $1/page. |

| We will ask you to arrive 30 minutes early to complete “New Patient” history and insurance forms. |

| Prior to any exam or procedure, a full explanation will be given to the patient so they will know what to expect. |

| Different test take different amounts of time. Patients will be informed of when their test results will available and how they will be contacted. |
